The Health Data Cycle – Prognosis of Functional Independence after Acquired Brain Injury (Ph.D. Project)

The aim of this WP is to optimize rehabilitation after an acquired brain injury by empirical estimation of the rehabilitation prognosis. The central part of the project is the development of a prognostic model delivering valid and reliable prognoses for the most likely degree of personal independence for individual patients.

This tool may support the clinical staff in expressing the expected prognosis to patients and their relatives, and inform the latter of the most likely course of rehabilitation. The tool will be developed using routinely collected health data, i.e. patient and treatment characteristics. In addition, the average prognosis for different levels of functioning and mortality after discharge from brain injury rehabilitation will be derived.

Clinical Business Intelligence and patient-reported outcome measures as decision support systems in neurorehabilitation

The purpose of this work package is to improve neurorehabilitation at HNRC and the VCR. The focus is on a) optimizing the effect of the treatment and b) streamlining resource allocation by establishing an innovative research infrastructure. The WP entails merging of the existing local data warehouse infrastructure with patient reported outcome measures (PROM). Daily updates of individual responses and implementation of advised actions will be part the WP.

At present, there is no systematic patient-centered follow-up after the end of hospital-based neurorehabilitation, neither nationally nor by HNRC or VCR. An aim of this work package is to identify individuals for whom life after rehabilitation after spinal cord injury (SCI) or acquired brain injury (ABI) does not proceed as expected.

We want to use the established research infrastructure to monitor functional impairments and how the individuals cope with and experience life after SCI or ABI. Through the development of the infrastructure for clinical implementation, we can advise former patients to establish contact with the healthcare system in the future, if necessary.

Another part of the WP is to investigate the effects of these new opportunities. The new White Paper on Rehabilitation recommends such efforts, where research, quality assurance and optimization of data registration are closely linked.

Optimizing the rehabilitation effort and resource allocation is sought by developing automatic real-time audits and investigating the effect of these on daily clinical routines. The real-time audits may prove manual audits obsolete and ensure timely clinical assessment as demanded by local or national guidelines for treatment in the hospital setting.

The vision for the WP is that we can register, merge and visualize data from all interested patients with spinal cord injury or acquired brain injury in the Central Denmark Region, including PROMs using. This may be accomplished using either Ambuflex or REDCap as data collection platform.

In addition, personal health data from both the electronic hospital records and the municipal record systems will be included, too. Hereto, a collaboration with the CDR’s research project “Tværspor” is established. This direct link between data sources will provide an unique opportunity to investigate research questions about post-rehabilitation trajectories of the population of interest.

Finally, it offers the ultimate opportunity for relevant clinical development that may put the CDR at the forefront, both nationally and internationally, in terms of research-based work with patient data and swift implementation of results and tools into the clinical reality.

Data capture and automation of clinical administrative tasks

The project includes the development and implementation of an automation of a clinical care score. The score is an expressing of the load of care tasks in the clinic.

Relationship between functional level at admission and BMI change in patients with spinal cord injuries: A retrospective observational study

The project uses data from the electronic hospital records via the BI data warehouse to investigate BMI change over time and correlations with functional level.

Use of psychotropic drugs in patients with acquired brain injury during admission to a neurorehabilitation hospital

The aim of the project is to elucidate the use of psychopharmaceuticals in the rehabilitation of patients with acquired brain injury. This includes investigating differences in drug use between patients with traumatic brain injury and patients with brain injury from other causes. The dataset will be prepared by members of the group and extracted from the BI data warehouse.

Risk of pneumonia during inpatient rehabilitation in patients with stroke, traumatic brain injury and subarachnoid hemorrhage

The aim of the project is to identify risk factors for pneumonia and pneumonia related mortality in patients admitted for rehabilitation after brain injury. The dataset used to identify risk factors is created on the basis of the local BI database enclosed in the CRD BI data warehouse. Quantitative exploration of the documentation practice will be delivered and facilitated by members of the BI group.

Incontinence among adults with acquired brain injury admitted for rehabilitation

The aim of the project is, to shed light on the documentation practices concerning incontinence and urinary tract infections and furthermore to estimate the prevalence of urinary tract infections among patients hospitalized for rehabilitation after brain injury.

The Clinical Academic Group on rehabilitation, focus on nutrition

The project is supported by CRD and covers a) documentation practice for nutritional status b) development and evaluation of a new documentation practice. Quantitative exploration of the documentation practice will be delivered and facilitated by members of the BI group.
